Player Certification {PLEASE READ!!!}
This page provides information about Certification Requirements if you are certifying a player in the SAM JACKSON TINY MITE LEAGUE.
ATTENTION!!! Once the season starts, certification will be shut down every Friday night by 11:59 PM. It will reopen by Monday morning 6 AM. The form may be left open, but the certification team will not resume until the following Monday. Please make sure your athlete is certified before gameday as this league has open registration until June 13th.
2026 CERTIFICATION
This year certification will be completely electronic. Please read below to learn what the qualifications will be to have your athlete certified with the league. Directions are in full detail below.
You will be required to fill out the form on the following page. There will only be one form per athlete.
You need to upload a Color photo of the following. The child's birth certificate, the parent/ guardian drivers license/state ID (If the child has state ID or a passport, NO parent ID is needed) and a headshot photo of the child. If the child does not have a state ID or passport, parent MUST upload their ID and their name MUST match what's on the child's birth certificate

Photo Basics

Submit one color photo, taken in last 6 months.

Use a clear image of your face. Do not use filters commonly used on social media.

Have someone else take your photo. No selfies.

Headshot photos only. No full body photos

Take off your eyeglasses for your photo.

Use a white or off-white background without shadows, texture, or lines.

You may not wear hats or head coverings, except for religious or medical purposes.

No other person is in the photo, the child is facing the camera with his eyes open.
When someone takes your athlete's photo, you must remove your face covering or mask so your full face is visible and the face covering and mask do not block portions of your face. We need to see your full face to establish your identity.
Please do not take a selfie because they are often the incorrect size and have the wrong head position for acceptable photos.
ATTENTION:
If your photo does not resemble the one pictured below, your child will not be certified and will not be entered into the database.
Age Verification
We will be authenticating athletes’ names, ages, by using a variety of documents to verify ages including state ID’s, passports, or parents driver’s license, birth certificates with athlete's blacked out social; while collecting parent contact information, and liability waiver consent.

Our Certification team's system will generate “team links” which are provided to coaches/admin after registering the team and paying league fees. Coaches/team mom then simply use the digital cards provided for check in on game day. Athletes are placed on rosters once their age has been verified.
The purpose of this is so that we....

No longer having to to spend time filling in extensive roster forms, they simply take the link provided and provide it to their team’s athletes/parents to complete in under 5 minutes.

Coaches no longer have to bring folders with sensitive personally identifiable information to events. All user data is captured and encrypted to ensure privacy.
We ask that you please upload clear and concise photos. only upload original COLOR documents. No black and White copies will be accepted. In the event our team cannot properly validate any of your uploaded documents, you will be contacted via email or by your organization.
Certification will stay pending until the athletes documents can be verified.
Once verified you may be asked to show actual documents to the league commissioner only. No-one other than league officiants are allowed to require you to provide any further documentation.
